Title:
ICL Explantation and Phaco Surprise
Abstract Body:
Purpose: To describe management of unexpected zonular weakness during ICL explantation and phacoemulsification in a high myope. Methods: A 36-year-old, 12 years post bilateral ICL implantation, presented with BCVA 6/15 OD, myopic progression, OD axial length of 34.09mm, visually significant cataract, and near-absent ICL vault. During ICL explantation, marked bag mobility and a visible equator indicated generalized zonular weakness. The ICL was removed, a CCC created, and soft nucleus prolapsed. Bag instability was controlled with capsular retractors, allowing safe nucleus aspiration and cortical clean-up. A capsular tension ring was placed, and a three-piece IOL was implanted in the sulcus with optic capture. Results:Surgery was completed uneventfully with good visual recovery. Conclusion:High myopes undergoing ICL explantation may harbour occult zonulopathy; timely use of capsular hooks, CTR, and sulcus IOL with optic capture can offer intrasurgical control and a successful outcome
