Seeing the Unseen: Intraoperative OCT Unveils hidden zonular weakness during cataract surgery
Abstract Body:
In an otherwise routine cataract surgery on a 56-year-old female, the initial steps (capsulorhexis, nucleus removal, cortical clean-up) proceeded uneventfully. Intraoperative OCT revealed subtle signs of localised zonular weakness (wavy, curled capsular bag) which would have been hard to detect otherwise. A capsular tension ring (CTR) was inserted promptly to stabilise the bag, demonstrating how intraoperative OCT can enable early detection and timely management of zonular instability
