Tenon's Tectonic Rescue: A Simple Option for Tectonic Restoration in Corneal (Re)perforations
Abstract Body:
In this video, we demonstrate Tenon’s patch graft as a simple, low-cost option for tectonic repair in small–moderate corneal perforations. We present a 42-year-old male with a leaking post-cyanoacrylate glue corneal perforation and shallow anterior chamber managed successfully with a Tenon’s patch graft. Tenon’s fascia is harvested through a small inferonasal conjunctival incision (which is also used to administer anesthesia), then packed into the perforation after removal of the pseudocornea; fibrin glue is optionally used as a scaffold. Compression sutures are placed to secure the graft, followed by air injection to reform the anterior chamber and reposition the incarcerated iris. At one month, the graft is well integrated, the anterior chamber is deep, and the globe remains tectonically stable. This technique avoids the need for donor cornea or specialized tissue/instrumentation and is useful in resource-constrained settings or when urgent globe-saving intervention is required.
