Study Design: A prospective, non-blinded, randomized controlled study conducted at a tertiary eye centre in India.
Purpose: To compare the clinical outcomes of intrastromal keratoplasty using biosynthetic cornea versus human donor cornea in advanced keratoconus.
Methods: Treatment-naïve patients (≥18 years) with advanced keratoconus were randomized into two groups: Group 1 (human donor corneal lenticule, n=10) and Group 2 (biosynthetic corneal lenticule, n=10). Efficacy was assessed by changes in thinnest pachymetry and maximum keratometry (Kmax). Safety was evaluated by monitoring post-operative adverse events.
Results: At six months, thinnest pachymetry increased significantly in both groups (Group 1: 346.8 ± 40.82 µm to 486.7 ± 15.7 µm, p<0.001; Group 2: 366.2 ± 31.3 µm to 548.3 ± 59.1 µm, p<0.001), with a greater increase in Group 2 (p=0.005). Both groups showed a comparable decrease in Kmax (p=0.44). No adverse events were reported.
Conclusion: Femto-second assisted intrastromal implantation of biosynthetic cornea is a safe and effective alternative to human donor cornea for advanced keratoconus.
