Title:
Beneath the Retina: A Battle against Bleed
Abstract Body:
A 67-year-old female presented with massive subretinal hemorrhage in the left eye due to PCV, resulting in hand movement vision and altered vitreous hemorrhage. A 25-gauge pars plana vitrectomy with posterior vitreous detachment and core vitrectomy was done. Subretinal tissue plasminogen activator(tPA) was injected to promote clot breakdown, followed by fluid-air exchange and silicone oil tamponade to stabilize the retina and assist blood displacement. Despite this, there was worsening hemorrhage, increased clot burden, and retinal detachment under silicone oil. A second surgery included silicone oil removal, limited temporal retinectomy, subretinal clot evacuation, ILM peeling under perfluorocarbon liquid, and reinjection of silicone oil. This case underscores the complexity of managing massive subretinal hemorrhage in PCV, particularly when primary intervention fails. Techniques such as retinectomy and subretinal clot evacuation can offer favorable anatomical and functional recovery.
