Abstract Body:
TITLE: Is It Always Necessary to Apply Botulinum Toxin into the Lower Facial Muscles in Hemifacial Spasm? Purpose: To debate the controversy regarding application of Botulinum Toxin in lower facial muscles in cases of Hemifacial spasm. Methods: We are discussing a case of 35-year-old lady with C/O of left hemifacial spasm for last 1 year. MRI brain was normal. After careful evaluation, we injected 2.5 Units in each 4 sites in periocular orbicularis oculi. We waited for 2 weeks for peak action of Botox. After 2 weeks again 3 spots in lower facial muscles were injected with Botox, each with 2.5 units. Results: Patient did report marked improvement in peribuccal spasms, yet it was persisting after periocular orbicularis oculi injection. Conclusion: BTX application to lower facial muscles might not be necessary in patients with mild lower facial involvement. In case of more severe spasms involving lower facial muscles, treatment targeting the lower facial muscles is recommended.
