Title:
THE HALO SIGN- A FRAGILE BALANCE
Abstract Body:
Aroutine case was posted for phacoemulsification.Following initiation of the procedure and successful capsular staining with trypan blue, a subtle but distinct ‘halo sign’ became apparent. This was visible for approximately 4 clock hours, correlating with an area of zonular deficiency. The halo appearance results from the loss of zonular tension, allowing posterior displacement and increased reflectivity of the anterior capsule in that segment. This sign serves as an intraoperative indicator of zonulopathy—one that may not be appreciated during preoperative slit-lamp examination, particularly in cases without obvious lens decentration or phacodonesis. Despite the zonular absence in the affected quadrant, the remaining zonules appeared intact and supported successful completion of the continuous curvilinear capsulorhexis (CCC). The halo sign represents a subtle yet valuable cue, aiding surgeons in early detection of zonular compromise and guiding intraoperative strategy."
