This surgical video titled "Pupillary Cerclage: A Beginner’s Guide" demonstrates pupillary cerclage in a 21-year-old male with a history of ocular trauma 10 years ago, leading to a traumatic dislocated cataract and mydriasis. The patient underwent CM-T Flex SFIOL implantation, followed by pupillary cerclage to address persistent photophobia. As this was the surgeon’s first pupillary cerclage, the video highlights key technical points to aid beginners: (1) avoid excessive iris traction to prevent iridodialysis and secondary glaucoma, (2) place sclerotomy ports neither too close—making needle handling difficult—nor too far, which may cause visible sutures due to iris tissue bite gaps. The aim is to provide practical insights and prevent complications, helping novice surgeons approach pupillary cerclage with greater confidence.
