Curtainless Windows: Surgical Challenges faced in Aniridia with Mature Cataract
Abstract Body:
A 45-year-old woman with a family history of congenital aniridia and severe ptosis presented with progressive visual loss and the fear of losing even the faint light she could still perceive. Examination showed bilateral aniridia, dense mature cataracts, nystagmus, severe ptosis, and corneal pannus, restricting vision to PL+ with accurate projection. Posterior segment view was impossible. With the aim of restoring some functional sight and easing debilitating photophobia, we planned staged surgery: the right eye underwent lens extraction with posterior chamber IOL and aniridia ring implantation, and the left eye received cataract extraction with IOL alone. Poor red reflex, absent pupillary support, and hazy cornea made every intraoperative step demanding. Yet, with determined modifications, surgery was completed safely. Her vision improved from PL+ to 3/60—numerically modest, emotionally transformative. This case truly reflects: when the going gets tough, the tough get going.
