When the Window of Light is Wide Open: Managing Pre-Existing PC Rent
Abstract Body:
This surgical video demonstrates managing a traumatic cataract with a pre-existing PCR in a young male patient, culminating in successful in-the-bag implantation of an EDoF IOL. Rather than intraoperative escalation, the strategy emphasized meticulous preparedness and strict adherence to surgical fundamentals. The video highlights controlled phacodynamics within the safe zone, deliberate chamber stabilization, and judicious use of a viscodispersive agent to coat and tamponade the posterior capsular rent. Fluid–air exchange was effectively employed to maintain anterior chamber depth and capsular integrity. Respecting tissue behavior and avoiding unnecessary manipulation eliminated the need for anterior vitrectomy, allowing a well-centered in-the-bag IOL implantation. This case reinforces a critical surgical philosophy: in complex situations, adaptability combined with disciplined basics often delivers superior outcomes—sometimes, doing less truly means achieving more.
