Phaco Surgery After RK With Zonular Instability
Abstract Body:
This surgical video demonstrates phacoemulsification in a post–radial keratotomy (RK) eye complicated by three major challenges: multiple RK incisions affecting wound construction and chamber stability, significant zonular instability, and intraoperative loss and retrieval of a capsular tension ring (CTR). The video highlights modified strategies for safe incision planning, gentle nucleus management with minimal zonular stress, and controlled retrieval of the dropped CTR. Key surgical pearls for maintaining capsular integrity and achieving stable, centered IOL implantation are presented. With careful planning and adaptive technique, complex post-RK eyes with compromised zonules can still achieve successful outcomes.
