Trouble of the bubble
Abstract Body:
Purpose: To present a video demonstrating refractive lens exchange with PCIOL showing difficulty in cortical matter removal due to air bubbles going behind the posterior capsule onto the anterior hyaloid face. Methods:42-year-old male with a refractive error of -10DS -2.5C at 90 degrees in the right eye underwent refractive lens exchange. An EDOF toric intraocular lens of 13.5DS-3.5DC(Axis 94 deg.). Surgery was uneventful until the step where air bubble went behind posterior capsule onto anterior hyaloid face through zonules, making further surgery difficult. Glow became dull, due to which cortical matter removal became difficult. Above video demonstrates maneuvers done to remove cortical matter. Results: On POD1, Visual acuity was 6/9 with near vision N10 and intermediate vision N18. IOP was 16 mmHg. On fundus examination, an air bubble was present in superior 1/5th of vitreous cavity. On POD3 , UDVA -6/6, Near vision N10. Conclusion:Patience and presence of mind are key takeaways.
