Triple Procedures - Challenges During Lens Management
Abstract Body:
Study Design: Educational surgical video compilation of complex keratoplasty with cataract extraction & IOL implantation. Purpose: To demonstrate lens management strategies during complex keratoplasty, improving intra-operative decision-making in combined cornea–lens surgeries. Methods: Challenging PK/DSEK cases with cataract extraction & IOL implantation were analyzed. The video shows stepwise approaches for capsulorhexis extension, positive pressure, decentered IOLs, subluxated mature cataracts and eyes post perforated ulcer. Scenarios including failed grafts with cataract, broken IOLs, aphakia, and vitreous prolapse are demonstrated. Results: Reproducible maneuvers improve visualization, maintain chamber stability, and support safe IOL placement. Strategies for weak zonules, poor red reflex & optimal IOL fixation are highlighted. Conclusion: “When the going gets tough, the tough get going!” This video offers practical guidance for managing complex keratoplasty with lens pathology.
