Traumatic Cataracts Unlocked: Surgical Excellence in Action
Abstract Body:
Purpose: To highlight surgical precision and decision-making across five distinct trauma-related cataract scenarios. Methods: A video case series demonstrates progressive complexity — from a classic rosette cataract with zonular loss, to posterior capsule fibrosis, to pre-existing posterior capsule defects, bisected lens, and frank posterior capsule rupture. Each case incorporates tailored techniques including capsular tension devices, Cionni-assisted phacoemulsification, pupilloplasty, sulcus IOL, pars plana vitrectomy, and staged visual rehabilitation. Results: All eyes achieved anatomical stability with well-centered visual axes. Vision was restored either immediately through in-bag or sulcus IOL implantation, or secondarily in the pediatric aphakic trauma case. Conclusion: Surgical excellence in traumatic cataracts lies in recognizing unique anatomical challenges and applying individualized solutions — converting complex trauma into reliable and predictable visual outcomes.
