Paracentesis turns Problematic : A rare post cataract fistula
Abstract Body:
Study Design: Single-patient observational case report of a rare postoperative complication after cataract surgery. Purpose: Evaluate the role of AS-OCT in diagnosis and management of chronic stromal fistula leading to persistent ocular hypotony. Methods: Clinical examination, and AS-OCT were used to characterize the fistulous tract. Multiple conservative measures including compression sutures, and tissue adhesive were attempted. Due to recurrence, full-thickness corneal patch grafting with excision of the tract was performed. Results: Conservative treatments provided only temporary closure, with leak recurrence within days. Surgical patch grafting achieved complete and sustained wound sealing with resolution of hypotony. Conclusion: Chronic stromal fistula after cataract surgery is rare and may resist conservative therapy. AS-OCT is essential for accurate diagnosis and targeted intervention. Definitive patch grafting offers reliable long-term repair.
