Managing a Stuck Toric IOL Using a 2.2 mm Microkeratome in Cataract Surgery
Abstract Body:
A 65-year-old patient underwent phacoemulsification cataract surgery in the right eye. A toric IOL was selected to correct the patient's pre-existing astigmatism. During insertion of the toric IOL, the trailing haptic became unexpectedly lodged within the inserter cartridge while the optic was already inside the eye, preventing proper deployment. Attempts to release the lens were unsuccessful, and further manipulation of the inserter raised concerns about damaging the IOL or the insertion system. Intervention: To resolve the situation, I decided to utilize a 2.2 mm microkeratome to open the tip of the inserter cartridge. The blade was carefully advanced to open the cartridge and release the toric IOL without compromising its integrity. Once the IOL was freed from the cartridge, the insertion system was carefully manipulated to position the lens into the capsular bag in the desired orientation.
