Second chances: Phaco after TPK
Abstract Body:
This video will demonstrate phacoemulsification in an eye that has undergone therapeutic penetrating keratoplasty (TPK) — one of the most demanding scenarios in cataract surgery. A farmer underwent TPK for a perforated corneal ulcer. Subsequently, he developed total white cataract. To compound the problem, there was 3-quadrant peripheral anterior synechaie, and occlusio papillae. The video will demonstrate how each challenge was addressed from placement of incision, getting access to the cataract , disassembling the total cataract, and choosing the intraocular lens power. And in the process, protecting the delicate graft endothelium, and preventing allograft rejection. Vision gained after the surgery was unaided 20/100 improving to 20/50 glasses (Plano with -2.50 dcyl x 180), and the graft remained clear.
