Abstract Body:
Complications can appear without warning, for the experienced and the new surgeon alike. A posterior capsular rent can turn a routine cataract into a moment that demands calm recognition and disciplined action. In this case, early identification shifted fear into focus. Chamber stability was maintained with viscoelastic, instruments were exchanged deliberately, and cortical cleanup was assisted with a meticulous anterior vitrectomy to avoid vitreous traction and further complications. Stepwise management, maintaining the anterior chamber – careful exchange of intruments through the side ports , a clean vitrectomy and sulcus placement of a multi piece IOL. Further confirmation of vitreous remnants with triamocinolone acetate staining and careful vitrectomy ensured a good surgical outcome .The lesson is simple: a rent isn’t a crisis—it’s a test of composure, patience, and trust in fundamentals.
