Abstract Body:
Posterior capsular rupture is one of the most dreaded complications during cataract surgery. This case describes an intraoperative posterior capsular rupture encountered during early phase of my phacoemulsification training, where continuation of phaco becomes challenging. The complication was managed by injecting a dispersive ophthalmic viscosurgical device (OVD) to maintain anterior chamber stability, followed by modification of phaco settings to safely complete emulsification of nucleus . Irrigation and aspiration of cortex was done carefully .Injection Triamcinolone acetonide was given to highlight vitreous strands, and a bimanual anterior vitrectomy was performed to clear the prolapsed vitreous. A sclero-corneal tunnel was then created, and a PMMA intraocular lens was implanted in the sulcus. On post op day 1 patient acheived vision of 6/18 .With timely and appropriate management, even such complications can result in good postoperative visual outcomes.
