Taming the slipping sun: CTR the Unsung Hero in a case of Cataract Surgery with Iris & Lens coloboma
Abstract Body:
Purpose: To highlight the role of capsular tension ring (CTR) in cataract surgery with iris & Lens coloboma and pre existing zonular weakness. Methods: 44 year old female presented with bilateral defective vision (RE>LE) for 2 years, with corneal diameter of 10*10.5mm, inferior iris & lens coloboma, advanced immature cataract in RE (vision CF 1m) & NS2+PSCC cataract in LE (vision 3/60). B scan revealed retinochoroidal coloboma not involving macula. RE was operated under guarded prognosis. Intraoperatively, lens subluxation& phacodonesis were noted. Capsulorhexis was completed with Utrata forceps, followed by immediate CTR implantation. Phacoemulsification was performed using direct chop technique with minimal zonular stress & a foldable IOL was implanted in the bag Results: On POD1, RE vision improved to 6/18 with stable IOL. Conclusion: Judicious CTR use stabilizes the capsular bag, minimizes intra operative complications, & ensures favourable outcomes, reducing secondary IOL need.
