Release, Recess, Reset: Transconjunctival Lower Eyelid Retractor Recession with Spacer
Abstract Body:
Lower eyelid retraction contributes to lagophthalmos and vision-threatening exposure keratopathy. Weakening of the pull by the unopposed lower eyelid retractors by recession is analogous to weakening of the levator to correct upper eyelid retraction and improve lagophthalmos. Here we describe lower eyelid retractor recession with a scleral graft as a spacer in a 24-year-old young male with 2mm inferior scleral show and 1mm lagophthalmos in the right eye. He is a known case of bilateral thyroid eye disease in quiescent stage for 18 months. The video focuses on the tips and tricks to identify the appropriate surgical plane to locate the lower lid retractors, the tips to decide the amount of defect to be created for the spacer and the ergonomics of using the globular contour of the scleral graft. At 6- months follow up the patient has excellent aesthetic and functional improvement with no inferior scleral show or lagophthalmos and symblepharon.
