Masked Mafia: An Orbital Epidermoid Cyst Masquerading as Sixth Cranial Nerve Palsy
Abstract Body:
A patient presented with diplopia, left esotropia, and limitation of abduction, and was referred as a case of sixth cranial nerve palsy. Neuroimaging of brain done elsewhere was reported as normal. However, detailed ophthalmic evaluation suggested a restrictive etiology rather than a neurogenic palsy. MRI orbit revealed a well-encapsulated lacrimal gland mass indenting the adjacent lateral rectus muscle. The patient underwent complete excision of the lesion through a lateral orbitotomy approach. Histopathology confirmed an epidermoid cyst. Postoperatively, the patient achieved orthotropia with complete restoration of lateral gaze and resolution of diplopia. This case highlights the importance of clinically differentiating restrictive strabismus from true lateral rectus palsy and emphasizes the role of orbital imaging in atypical presentations.
