Persistent corneal edema following cataract surgery is commonly attributed to endothelial decompensation or Descemet’s membrane detachment (DMD). We report a diagnostically challenging case referred for a second opinion with a prior diagnosis of DMD and advice for air descemetopexy. On detailed slit-lamp biomicroscopy and anterior segment evaluation,presumed “detachment” was identified as an anterior capsular flap adherent to the corneal endothelium, mimicking DMD. The misdiagnosis highlights the importance of careful morphological assessment and avoiding premature intervention. The capsular flap was meticulously removed, leading to rapid resolution of corneal edema and restoration of corneal clarity. This case underscores a rare but significant masquerade of anterior capsular remnants as DMD, emphasizing the need for critical evaluation before proceeding with invasive procedures.
